2009 ifa Insurance

Teams can purchase insurance from ifa
for the flat rate of $120.00 per team for all age groups
Teams must be registered with ifa and have submitted their roster
in order to purchase insurance

2009 ifa Roster for DFS Events

ifa insurance can be purchased through Dennis Fong or Bob Regpala
If purchasing through Dennis, fill out the form below.
You can pay by check or credit card - Coverage will not begin until
funds are received. Credit Card payment is the quickest means of
obtaining coverage. There will be a $2.00 service fee for credit card charge.

Additional Insured Certificates
Many teams require a certificate naming a property owner or entitiy
as Additional Insured for practice fields. There will be an extra cost
of $25.00 for each Additional Insured certificate.
